Ok It's not workflow as it has not been changed in several years. Must be a patch they forgot to include in their latest build.

Yes. It started when we upgraded to 7.0.1, and it happened all the time, not just occasionally. It went away when we upgraded to 7.1. (We are on a RH Linux ARS server.)
