Atul, I just did this myself. There are two fields, 14506 and 14507, that I had to add to my request form and to the join forms. You can find these two fields on the "AP-Sample:Lunch Scheduler" form.
As Jarl said, this looks like a bug to me. Either the documentation needs to include these two fields or the workflow needs to not include them.
