Atul, You would be better off re-installing the approval server and specifying to install the Sample applications. This will install both the "AP-Sample:Lunch Scheduler" form and the "AP-Sample2:Get Agreement" form (among others). That will eliminate the error you list below and allow you to copy the two fields to your request form.
If it is not possible to re-install the approval server let me know and I can get you the def to you.
