Misi, I have experienced some of the same problems you have had. The one that really surprised me was copying a form WITHIN the application and the Admin Tool reporting the permissions had been removed. However, when I went back into the form, all roles and permissions remained. I have not, however, had the pleasure of receiving the "form does not exist on server" error. I'm sure it will be coming soon as I continue working in deployable application.
