Also, if Norm's suggestions don't resolve your issue (correct those first), You may want to check your default Plugin Server timeout.
How many LDAP records do you expect to be returned via your vendor form? If it is a large amount, your Plug-In server may be timing out before it can load the connector and related tables. In this case, you may need to set the Default Plugin Server timeout to a higher value. I think the default is 60 seconds. We had a similar issue, where we had over 100K LDAP records, and had to set the timeout to 500 before we could even create the vendor form.
