I'm pretty sure that this is a bug.  Don't have time to find the # for you,
but check support website (we have the same issue-makes for a nasty log).

Usually turning on your SQL logs can tell you the cause of such errors..
turn them on and look at the last statements that were successfully executed
by the database before the database rolled back transactions.
 
Trigger the action or actions that cause that error as soon as you turn on
your SQL logs and then filter the SQL logs by the user that exectued the
actions that triggered the error.

I am noticing an error message in the arerror.log file. The message is as

Ok I understand what the entry means, what I cannot find is, what is causing
this to happen. Obviously some workflow/process is trying to create entries
in this form. But what process or workflow? does anyone have any idea?
