I found the solution using BMC's knowledge base (which isn't working with IE 6 for me but is working with Firefox if anyone else is having issues.) I'm not happy that I had to create a field in the reserved range of field ids, but it worked and that is what matters for now.
In this case, the customer had created a field in the custom range (536870917 field ID) on the application form and had added it to both of the joins for the approval server - the two way join between the application form and AP:Detail and the 3-way join between the application form and AP:Detail-Signature. We restarted the approval server and set the Definition Check Interval to 0, however, there was no change. What fixed this was to use a field in the Remedy Reserved range. Customer created a field ID in the reserved range and used this for the first approver and this worked.
