Server name is the physical name of the database server (AND NOT the AR System server unless you have the AR System and the database installed on the same machine for whatever reason.) Database user name would be ARAdmin if that has not changed during the install of the AR Server and the password if it had been left to default would be AR#Admin#. If for the purpose of reading your data, your DBA's have created a separate user, with read access to the database, use that user instead.
