Tim, This happened to me once - no error nothing - just blank fields..
Turned out my system was missing work-flow - the work-flow supporting that Administration page was missing.. They were all Active Links.. If you have a working system, compare the active link list on the working system for that form to the list on the non working one and if there is a discrepancy, sync them up. Make sure both the servers are the same version to the last patch though..
