As a follow-up, this is now resolved for us. It's described in KB 20014158 and 
is a known defect.  The KB includes a zip file
with a new HPR_Incident_Print.rpt file.  That fixed it for us. I imagine that 
there are several possible causes, a number of
which were described as this thread went along.....
