We do not have any special template for Remedy. I think that creating an special template for Remedy can lead to use a bad practice: Use technical language on requirements. Requirements must be understood by all stakeholders. I prefer to use sentences like "When pressing this button, the date will be uptaded", than "Create an active link to be fired on pressing the button that updates the Date field". What I have is an internal analysis document that specifies the objects to be modified, created or deleted.
