We tested by bypassing the proxy using the IE "bypass proxy server for local 
addresses" setting as well as entering some exception addresses and by doing so 
worked.  I did log a call with BMC and they stated, what I mentioned that only 
the mid-tiers must do the caching.  We initially thought it has something to do 
with the mid0tier cache or the load balancers though.

Thanks for your help, the solution was that the proxy the users were routed 
through was caching the mid-tier url's and according to BMC the mid-tiers is 
the only device that is supposed to handle the cache.

No problem.. If the toolbar is installed, click on the little button attached 
to the search button on the google toolbar, that has a little triangle like 
black icon on it representing a drop down.. (see GoogleToolbar-01.jpg)

Go to the Tools tab of the Toolbar Options that open on clicking on  that 
button, and make sure that the Pop-ip Blocker is disabled (see 
GoogleToolbar-02.jpg)

I had an issue with pop-ups recently, where it turned out that the google 
toolbar that was installed on IE, had pop ups within it disabled, that caused a 
any pop ups to close as soon as they opened. I was helped by a few members on 
this list to resolve that problem..

The cache is flushed on a regular basis.  We might have to do this again but 
then the cache might get corrupt to  quickly to often.

Have you tried to flush the cache of the midtier ? With my ITSM 7.6, the cache 
gets corrupted weekly, and I have similar behaviour with some forms.

Hi All,

We have a strange issue where when adding an attachment on Incident Management, 
the dialog (Attachment Popup) mess up the buttons.  This does not occur on all 
user pc’s just some.  We are running a load balancer and application servers is 
in server group.

App Servers
Windows 2008 R2
Mid-Tier 7.6.04 SP2
IE 8 & Windows 7

We have tried on one of the users to upgrade to IE9 but having the same issue.  
We also tried to clear the browser cache, cookies etc
