At this point my only suggestion is to activate the escalation logs to see what is really happening. It could be a lot of possible root causes, from another escalation creating those requests, or a time configuration on the server different from the client, or a busy escalation pool that delays the execution, or ... a lot of cases. turn on the escalation log, wait for a request creation, and see what happened at this time.
