Mark: I'm not sure if these are the OOB questions or not, but here's what we have:
1. Number of users impacted during implementation (Pick number ranges of your choice depending on number of users in the company - it might be good to map these values to those you use for Impact) 2. Does this change have potential to negatively impact IT Services (Y/N) 3. Is this change scheduled to occur between <insert standard business hours here> (Y/N) 4. Is there a documented backout plan for the change? (Y/N) 5. Is there a documented test plan for the change? (Y/N) 6. Has the change been communicated to the necessary parties (e.g. Service Desk)? (Y/N) Personally, I think question 2 could be clarified, because I read the question as "does this change have the potential to have a negative impact?", in which case the answer is always going to be Yes. But is it said negative impact gonna bring down a major service like Email, or are there other dependencies the said negative impact could affect like DB, server, network, etc.? Well, maybe not. The Change Management Risk Assessment , Risk Level - 1, -2, etc is not very user friendly so I was thinking I would make that field read only and force the user to answer the Risk Assessment questions to determine the risk. OOB the questions are not great. Does anyone have a list questions that they use and would be willing to share?
