I'm assuming you're running ITSM, therefore you must be usint the internal 
notification system (NTE); if this is the case you should look at the 
configuration of the messages and how they're being pushed to the NTE:SYS-NT 
Process Control form; there's the field Priority, if you make this "Critical" 
then once the message reaches the AR System Email Messages form, there it will 
place as Flag 0, which makes when Outlook receives the message appear with the 
critical icon. Either if you want to every single mail being dropped by Remedy 
has the critical flag on, just create a filter for AR System Email Messages 
form, to place as flag, 0.

I'd suggest you to look directly by using the configurations at NTE:SYS-Process 
Control; instead placing code to core forms.

Hi All – I have a request to have an email come out of the system with the High 
Importance flag in Outlook is this possible?  The emails are being sent with
