select "Document Objects". Select server Name and press Next At the "Select Objects" Dialog, add the form you want to document and press Next. On the output details dialog, Select Document fields, and Summary. For other options, select Regular. Save as HTML Press Finish. This will output 2 HTML Documents. The Field.html document should contain what you are looking for.
