There is the free ITSM 7.6.04 Forms and Fields Excel; spreadsheet which will give you the label for the null or English views, IDs, Names, as well as an interpreted "field type, limits, default values" from an OOTB stack install of ITSM 7.6.04.
If you need to generate it against your own server (because of customizations) then simply fill out the Meta-Update trial license, attend the "Live Install" session. In your trial you can run Meta-Schema against your server(s) and generate your own spreadsheets. Note that the spreadsheet is big: ~2.5 M fields.
