Hi, We did faced similar issue several times even on 7.0.03 environment. Removing application permissions and reapplying permissions resolves issue each time.

I've not seen this in 7.6.03 but did start to get a similar thing in 7.6.04 - to resolve it I removed their application permissions and added them in again.
