Nope...BMC doesn't provide the form, so it wouldn't be able to be overwritten....but depending on your version that you are upgrading from (lets propose some time in the future)...if you added your form as an 'additive' overlay to the application from 8.1 to say...8.5 (or whatever the next version is), then you will still have your form in the application even...but if that sort of granularity doesn't exist, then yes, you would need to re-add your form to the application, but still, your form should be in tact.
