Andrew, In addition to adding an overlay on the Form, you also need to overlay the field, and potentially the view as well.
Regarding why you can't attach via workflow....you can only 'change' the menu attached to a field via workflow, you can never add a menu...so if the form doesn't have a menu attached in the first place, you can't add one.
