You also should check and see if your server threads are overloaded for the thread the approval server is using. We had problems with the approval server and then moved it to a private queue and that helped a lot.

There is also an option that Approval Server to run on its own specific thread - Setting this thread can say for sure that the issue comes from approval server. You can get more information by switching on thread log + adding Copy-Cache-Logging:T in ar.cfg file.
