I saw this issue yesterday when I was trying to set up SSO on my midtiers. We 
are using a load-balancer as well, but the problem was not being caused by 
that. The problem is that  SSO is set up to use Areahub and two associated 
plugins, which are arealdap.dll and areasso.dll. It seems that Remedy is using 
a new authentication method for AREA and it conflicts with arealdap.dll. To 
resolve this issue I had to change the ar.cfg file and comment out this line: 
Server-Plugin-Alias: AREA AREA <servername> 
If you see the line Plugin: "D:\Program Files\BMC 
Software\ARSystem\arealdap\arealdap.dll"  in your ar.cfg and 
Server-Plugin-Alias: AREA AREA <servername>, then you could have a conflict  
that will end up causing you an ARERR 623 Authentication Failed Error. You may 
find that your AD account gets locked out as well because of this.
