Since you say it doesn't happen all the time you may want to double check the inbound source email and compare one that works from one that doesn't by doing the following:
In the Email Messages form Choose Display Advanced Options = Yes Go to the Advanced Options tab then the Attachment Alternatives tab The true source is in the Email attachment. Save it to Disk and open it with WordPad. You can then compare the From: values to see if there is a difference in the structure of one that works vs one that doesn't.
