The complaint is in the .log file left behind by dblatex (pdflatex).
You'll need to run dblatex with debugging on so that it keeps
its temporary output directory around:
a2x -v -k -f pdf --dblatex-opts='-d' asciidoc-test.txt
After running, it will print a message something like:
'asciidoc-test.pdf' successfully built
/tmp/tmpr28RWn not removed
Look in /tmp/tmpr28RWn/asciidoc-test.log for the warning message.

Yes it happens.
I suspect its just nobodies ever looked in that file before.
I'm not sure what the implications of changing the package name are,
better leave that to Stuart.
I don't have an answer off the top of my head, as far as I can tell it's benign.
