You'd probably need to hack the asciidoc.js, because it only works off the
toclevels passed to it when the asciidoc.install function is called on
document load.

I was thinking that you could add [float] to the appendix to take it out of
the structure, but it seems you can't set both float and a special section
type (like appendix).
