The gpl_v3.asciidoc document starts internally with level 0 headings.

There are some problems with this approach:

1. The resulting sect-depth exceeds asciidoc's builtin of sect4. I created 
a local html5.conf with a [sect5] entry to solve this.
2. I get a single   "section title out of sequence: expected level 2, got 
level 3"   error for the first sub-heading beneath Appendix
3. The TOC looks a _little_ quirky with the sub-headings beneath Appendix 
starting further in than they should.

I can live with these limitations for now. I'll consider other approaches 
if I start to tolerate this way less.
