Thats correct. For html produced from docbook I think the xslt can be set up to do that, see http://oreilly.com/catalog/xslt/chapter/ch05.html#t3. Such a piece of custom XSL can be invoked when processing via a2x (see http://www.methods.co.nz/asciidoc/a2x.1.html --xsl-file), but I am far from an XSLT expert, and doing it is left as an exercise for the reader.
