M ark, To control generation of the toc when producing PDF via FOP, you have to pass the appropriate XSLT parameter (or tweak the XSL that generates the FOP). I'm not sure what that parameter is, I'd have to dig for it. The AsciiDoc attribute doesn't get used in the XSL template currently.
