Thanks for these interesting explanations. With constraints from two different universes (docbook vs. common CSS frameworks) it's hard to respect all best practices as the natural solution for one feels kludgy in the other context.
Asciidoc may not be the only project to hit this "impedance mismatch". Perhaps it would be good to draw the attention of some people working on CSS and/or frameworks.
