Hi Using unconstrained quotes (i.e. `__emphasis__') produces the desired output.
I found that, in addition to the asciidoc tool, there is another tool, asciidoctor. Asciidoc and asciidoctor do not agree on how various combinations of quotes and emphasis should be handled. Attached is a small example AsciiDoc document and the output generated by asciidoc (version 8.6.9) and asciidoctor (version 1.5.2). In neither case do I specify command line options.
