I suspect they do in fact increment correctly, but your epub reader
isn't rendering it correctly and is cutting off the leading digit,
notice that part of the 0 of 10 is also missing.  You would have to
unravel the epub to find the HTML in there to check.  IIRC an epub is
a zipped directory so unzipping it and looking for the HTML file in
the tree should work.
