Hi,

As defined here http://asciidoc.org/userguide.html#X88 the
toc-placement and toc::[] macro only apply to HTML5 and xhtml11
backends because Asciidoc generates the output directly, the placement
and provision of tocs for docbook based toolchains is toolchain
dependent.  Docbook does not have any way of passing that information
to the toolchain so asciidoc can't control it.  So PDF puts it where
it wants to, and I'm sure the ebub xslt can turn it on, but I don't
know how, try http://sagehill.net/docbookxsl/TOCcontrol.html might
help.
