I build using these commands: a2x -fpdf -dbook toc_placement.txt a2x -fepub -dbook toc_placement.txt For the PDF I get: title page, then revision history, then TOC, "some text" page, followed by the chapters. For the epub I get: title page, "some text" page, chapter 1, chapter 2. No TOC!
