I have everything enabled except AV, so I can't directly attest to that either, except that in testing the AV introduced significant delays in how long it took a message to process.
Your system is not particularly robust, I wouldn't doubt that it might come across as crippling. Your mem is fine, and multi-proc isn't not helping. We do roughly 10k connections and 3K actual emails processed a day. I have added the clamd service following the Wiki and the clam perl module per the Wiki. The log shows File::Scan::ClamAV module version 1.8 installed and available.
