No but if you try and install the service and it is already installed it
will not let you.

No - I believe Rick did this so it could be used as an upgrade install

It has a bootstrap setup.exe that runs the code for getting Perl.  It
also has an .msi  why it is so big? The installer was built with MS
Visual Studio need I say more? :-)
