Are you running *sender* address verification for all mail? This is 
going to cause you a lot of problems (regardless of whether or not it is 
causing this specific problem)...

You should only enable 'sender address verification' for your own domains.

*Recipient* verification, however, is the most important check to 
enable, and is enabled by default.
