Well, basic file type blocking may help in blocking executable files, scripts etc. But can have undesirable side effects (.exe zipfiles will be blocked for example). I have it turned on though since I believe executable files don't belong in an attachment (unless zipped or something). The more links in the chain you add the higher the chance one link fails and email delivery is affected. Plus you will also increase the chance of false positives. Not saying that using clamav is bad or anything, just arguing about "can't have too many AV scanners".
