The MAIN problems in most of these cases is to obtain a CLEAR
view of the "network" (meaning number/location of clients and so on)
and gathering infos to understand if that network (and those servers)
are ACTIVELY sending spam and/or if any client has been "r00ted"
see, before starting a "cure" you'll have to learn as well as possible
all the "symptoms of the disease" then, having a clear picture of the
whole issue you may move on and take the appropriate steps :)

For example, the latest v2 has a quite cool mechanism to help
avoiding OUTBOUND spam (or at least getting ALERTS about
such an issue) ... but before jumping in barefoot it would be advisable
to gather as much infos as possible and getting a "grip" on the whole
issue; that would help a LOT

Also, and since you're talking about exchange, I'd suggest you to
pick this critter

and use it to load/parse/analyze the exchange logs (and the SMTP
ones btw); it may help you better understanding the terms of the issue
and possibly putting down a plan
