If OpenVPN (which uses a software network "adapter") fixes the issue, I have to think that one thing that should be looked at is network adapter settings. I can't think of anything else that would be different between scenario one for Doug (STARTTLS over the internet) and scenario two (STARTTLS over VPN), assuming I understand him correctly.
