Yes they are. And nobody seems to know how to diagnose it.

How is it possible with digital lines?
It basically isn't (there are a few exceptions like a bad speaker phone
causing echo or other oddities like that). Everyone forgets that the
OTHER END OF THE PSTN CALL is usually analog and therefore WILL generate
echo.
