Have you looked at vicidial? It has CRM integration that allows for grabbing Caller ID and bringing up the relevant record from the database. It also has a custom Web Form that you should be able to use if there's no existing entry for that particular number Contact me off list at [EMAIL PROTECTED] if you would like for us to set it up for you
