How does the company providing E911 services get address info for your
customer who is dialing 911? Don't they need access to your customer
database (most importantly, the table holding the address info specified by
the user as the physical address)?

You have to update them, just like your customers update you.

Most of them have some kind of database you integrate with or an interface where you can type them in. There are also some telephone-based services. Everything trickles down to essentially one or two companies which route the calls and maintain the real databases.
