This corrects some issues introduced in the responses to the CoreShowChannels
AMI command as well as adding documentation for the responses. The command in
Asterisk 12 was missing the following fields: Duration, Application,
ApplicationData, and BridgedChannel and BridgedUniqueID (replaced with
BridgeId).
