The VoIP clocks are not synchronized to the PSTN, to each other or to a common reference. There is bound to be a frequency difference between several devices. Over time this will lead to dropped packages. At present Asterisk MeetMe does not drop superflous samples, instead choosing to queue them. See bug http://bugs.digium.com/view.php?id=4252.
