Here's what I've been told to use and it works with teliax & Nufone:
exten => _1NXXXXXXXXX,1,SetCallerID(4025551212|a)
exten => _1NXXXXXXXXX,2,SetCIDName(NPI|a)
exten => _1NXXXXXXXXX,3,Dial(IAX2/teliaxout/${EXTEN})
The setcidname isn't really needed under most circumstances, but I
send it anyway for consistency.
