Using AVT in my sipura with above settings and it work fine going out the PSTN. There was an issue a while back with an older version of Asterisk with one of my providers but it has been fine since the upgrade. I also use ulaw for calls.

Apparently there is an rfc2833 issue and * is working on it??? Also it appears the codec used might be an issue. This is a serious problem in my book.
