Similar probles I had were fixed incrementing one of the timers, but if you have already done that, I have no idea of your problem, you require to debug the problem and try to find some consistence in the failures, find if the failure is on the Asterisk -> telco Link, or in the Asterisk -> meridian link? find if putting in loop your own asterisk still fails, etc etc.
