What kind of phone are you using? some devices which connect you pstn phone to voip network have their own special dtmf keys defined inside their configuration to perform special actions, maybe thats your problem. also check for your dtmf setting. dtmf settings should be same on both sides.
Also, if you're using SIP phones, check and make sure you've got the right signalling (rfc2833, inband).
