RTFRM:

http://docs.tzafrir.org.il/dahdi-linux/#_kernel_source_headers

(This is the README file in the dahdi-linux tarball)

A bit of information that is not there, and I wonder if it should:
Centos mirrors typically don't keep kernels for older Centos minor
revisions. That is: only the packages for the latest 5.x (and the latest
4.x, and 6.x) are included. kernel version 2.6.18-194.el5 is from Centos
5.5, IIRC. Either upgrade the kernel, or grab the older one from
http://vault.centos.org/ .
