This would seem not to be an isolated case. I am the author of the FWD Assistant, a GUI tool for MacOSX that automatically configures Asterisk for one or more FWD accounts using the IAX option. Over the last two weeks some users have contacted me reporting similar problems because they thought it was a bug in the assistant. While troubleshooting this, we found that calls made by another FWD subscriber using IAX would come in while calls made by another FWD subscriber using SIP would not. This led me to believe that there is a problem with handing off calls from FWD's SER system to FWD's Asterisk and since it is not the case with all accounts, I assumed there was a problem with the provisionsing process at FWD. I asked the users experiencing this problem to disable their IAX option at FWD, wait 10 - 20 minutes and then re-enable it again. This always fixed the problem. However, at least one user reported that he had to apply the fix again after a few days. I would have reported this to you earlier, but since you never responded to any mail before I asked one of the users to post some feedback on the FWD forums instead.
